Removal of a Basal cancer on the neck
Front line physicians will often times not feel comfortable doing biopsies or removals of lesions on the neck but these are very possible if you respect the surrounding anatomy, progress with appropriate caution and essentially remain superficial while ensuring proper margins. I generally find total removal simpler than an actual biopsy as you aren’t cutting into a cancer or trying to suture through it after the fact. In Canada, at least where I practice, you cannot get plastics or oncology to see these lesions without a tissue diagnosis which forces you to do one or the other. If you are comfortable doing a removal you can do one procedure, achieve clear margins (as we did in this case) and then you are entirely done.

Drainage of a deep abscess on the back
Переглядів 772 тис.Місяць тому
This one was a bit atypical in its depth and in the amount of content it held. It felt quite large on palpitation but even having said that, I underestimated how much volume this held. I did see this patient the following day and he felt drastically better and things were healing nicely.
Removal of a Basal Cell Cancer on the face
Переглядів 209 тис.Місяць тому
Ideally Basal Cell Cancers and Squamous Cell Cancers should be removed using a Mohs procedure. However, at least for BCCs, sometimes just removing it, and cauterizing the base will be enough to eliminate all the neoplastic tissue. This one healed quite nicely and didn’t require any further treatments.
Drainage of an infected cyst on the jaw line
Переглядів 1,4 млн2 місяці тому
This area was clearly infected which was obvious once this lesion was opened and pus began pouring out. Thankfully the anesthesia was able to work well enough that the patient was comfortable enough to tolerate this process. These are fairly straightforward as long as specific steps are always followed.
Drainage and likely removal of an infected epidermal cyst
Переглядів 282 тис.2 місяці тому
While most infected epidermal cysts cannot be removed properly, as the infection thins out the membrane, this one was intact enough that, fortunately, we were able to remove it all. I wish that this was commonly the case but it typically isn’t. Packing overnight usually isn’t overly uncomfortable to remove. Repacking and subsequent dressing changes would be, though, and this is why we typically...
De-roofing of nasal cyst
Переглядів 1,5 млн2 місяці тому
Small epidermal cysts or large milia (which are pilosebaceous cysts) respond very well to simple de-roofing. Enough capsule is compromised that the area involutes and the cyst resolves. This healed quite well and without recurrence.
Is this a pyogenic granuloma?
Переглядів 449 тис.2 місяці тому
This was referred to me as a pyogenic granuloma, for removal, but while this shared some features with a PG, it was not quite right and didn't respond to treatments as it should have. As a result a biopsy was undertaken and this turned out to be a squamous cell cancer. These have a slightly higher metastatic rate than basal cell cancers do, so it is important to make sure that all the margins a...
Removal of epidermal cyst on the ear
Переглядів 2,5 млн2 місяці тому
There is nothing complicated about this removal so long as one progresses slowly and consistently. Here, a small elliptical incision certainly makes the overall approach a bit simpler, but a linear incision would have been reasonable as well. While the cyst capsule was irritated a little, this essentially was removed entirely intact, guaranteeing that this particular cyst will not recur. Hope y...
Removal of Epidermal Cyst on Back
Переглядів 680 тис.3 місяці тому
This was a medium sized epidermal cyst that was a bit harder to remove than I would have thought initially. As a result, it was necessary to rupture the cyst capsule, to reduce the overall size of the lesion and also to increase the spacing within which I was working in. Also, because of the tight space, the lining of the incision was nicked a few times and needed to be cleaned up at the end. M...
Removal of a Pyogenic Granuloma
Переглядів 1,7 млн3 місяці тому
This was a bit messier than normal as I didn't have any assistants present on a Sunday morning. Regardless, this was pretty straightforward and this patient was pretty stoic, which helps a lot in kids. Pyogenic granulomas aren't worrisome but they can bleed a lot and be uncomfortable. They grow very quickly and at this size, will need to be removed and sutured closed. This went pretty well but ...

Doing a facial repair
Переглядів 37 тис.4 місяці тому
Facial lacerations are often passed along to ER as many front line physicians aren't comfortable dealing with them. However, so long as the tissues are appropriately apposed these should heal quite nicely. Certainly if the laceration is quite jagged then plastics may need to get involved but front line physicians should be able to determine when that is the case and otherwise, repair these.
Removal of Multiloculated Cyst and Scar Tissue on the Face
Переглядів 366 тис.4 місяці тому
This was a little bit complicated in terms of approach but not so much in terms of execution. While there were certainly a few cysts located along his right cheek, the bigger issue was the intervening tissue was quite firm and scarred down. This put us in the position where we were forced to consider removing the entire area if you want the inflammation to settle down. On the face, this can be ...

Had one of these. It hurt soo bad, thought I broke my tailbone. And then the procedure was painful as well. I screamed bloody murder in the doctors office. But once the pressure was released, it didn't hurt as much.
