This channel is for anyone who loves procedures and medicine in general. While this will certainly have an educational slant I hope that it will grow in numerous ways to be useful for the beginning as well as those with more clinical experience. I am a family doctor and adjunct professor with the local medical school. I have been fortunate to receive excellent teaching from plastic surgeons, general surgeons and dermatologists that has enabled me to acquire the ability to do numerous procedures. It has been my honour to learn from these great people and I hope to pass along some of what they taught me.

It’s sometimes a bit difficult working in this area. The tissues are a bit more elastic and that makes incisions and working with these tissues a bit more challenging. In this case, the membrane ruptured a bit but was still removed in its entirety.

This one was a bit atypical in its depth and in the amount of content it held. It felt quite large on palpitation but even having said that, I underestimated how much volume this held. I did see this patient the following day and he felt drastically better and things were healing nicely.

Ideally Basal Cell Cancers and Squamous Cell Cancers should be removed using a Mohs procedure. However, at least for BCCs, sometimes just removing it, and cauterizing the base will be enough to eliminate all the neoplastic tissue. This one healed quite nicely and didn’t require any further treatments.

This area was clearly infected which was obvious once this lesion was opened and pus began pouring out. Thankfully the anesthesia was able to work well enough that the patient was comfortable enough to tolerate this process. These are fairly straightforward as long as specific steps are always followed.

While most infected epidermal cysts cannot be removed properly, as the infection thins out the membrane, this one was intact enough that, fortunately, we were able to remove it all. Small epidermal cysts or large milia (which are pilosebaceous cysts) respond very well to simple de-roofing. Enough capsule is compromised that the area involutes and the cyst resolves. This healed quite well and without recurrence.

There is nothing complicated about this removal so long as one progresses slowly and consistently. Here, a small elliptical incision certainly makes the overall approach a bit simpler, but a linear incision would have been reasonable as well. This was a bit messier than normal as I didn't have any assistants present on a Sunday morning. Facial lacerations are often passed along to ER as many front line physicians aren't comfortable dealing with them. However, so long as the tissues are appropriately apposed these should heal quite nicely. Certainly if the laceration is quite jagged then plastics may need to get involved but front line physicians should be able to determine when that is the case and otherwise, repair these.
